There is a catch. Heart rate went up a little, more at higher doses. It peaked around week 24 and then eased off. And nobody knows yet whether retatrutide lowers heart attacks or strokes. That trial is still running.

Blood pressure in plain words
Think of your blood vessels as garden hoses. Your heart is the tap. Blood pressure is how hard the water pushes on the inside of the hose.
A reading has two numbers, like 120 over 80:
- The top number (systolic) is the push when your heart beats.
- The bottom number (diastolic) is the push between beats, when your heart rests.
Pressure is measured in “mmHg,” which stands for millimetres of mercury. You can just think of them as points.

High pressure matters because it wears the hoses out. Over many years, it raises the risk of heart attack, stroke and kidney disease. The tricky part is that it usually has no symptoms.
High blood pressure in Canada
High blood pressure is very common here. Statistics Canada measured the blood pressure of thousands of adults across the country.

Look at the last bar. Younger adults with high blood pressure are the least likely to know it, and only about half have it under control. Many people only find out at a pharmacy machine or a checkup.
Why weight and blood pressure are linked
Extra body weight tends to push blood pressure up. A few reasons, in plain words:
More to pump
A bigger body means more blood vessels to fill. The heart has to push harder to reach them all.
Kidneys hold salt
Extra weight, especially belly fat, nudges the kidneys to hold on to salt and water. More water in the hose means more pressure.
Stiffer hoses
Over time, high blood sugar and blood fats can make vessel walls stiffer and less stretchy.
Night-time breathing
Sleep apnea is more common with extra weight. Breathing that stops and starts at night raises pressure.
That is why losing weight is one of the best-known ways to bring blood pressure down. So any drug that causes big weight loss would be expected to lower pressure too. The question is whether retatrutide does anything extra.
What retatrutide is, in plain words
Retatrutide is an experimental peptide. A peptide is a short chain of amino acids, the building blocks of protein.
Your gut makes hormones that tell your brain and body when you are full and how to use energy. Think of them as three buttons:
- GLP-1: the “I’m full” button.
- GIP: a helper button for appetite and blood sugar.
- Glucagon: the “burn more energy” button.
Semaglutide presses one button. Tirzepatide presses two. Retatrutide is built to press all three. In its biggest trial so far, people in the top dose group lost about 28% of their body weight in 80 weeks. Our semaglutide vs tirzepatide vs retatrutide guide covers the differences.
What the trials found on blood pressure
Blood pressure was measured in every retatrutide trial so far. Two independent reviews have pooled the results.
Points = mmHg, compared with placebo. Bars scaled 0 to 12 points. Sources listed at the end of this guide.
In plain words: people on retatrutide ended up with blood pressure a few points lower than people on dummy shots. A drop of 7 to 10 points in the top number is about what many single blood pressure pills do.
In the 36-week Phase 2 trial in people with type 2 diabetes, the top number dropped by up to about 8.8 points. The 2026 review also found lower cholesterol and much lower triglycerides, which are another kind of blood fat.
People who cut back on blood pressure pills
One of the most telling results came from the Phase 2 obesity trial. Some people started the trial already taking blood pressure medicine.

These changes were made by trial doctors who were checking blood pressure closely. That is the point: when weight comes off fast, blood pressure medicine may need to come down too, and only a doctor should make that call.
Is it the drug or the weight loss?
This is the honest open question. Nobody has pulled the two apart yet.
- Big weight loss on its own lowers blood pressure. Retatrutide causes big weight loss.
- The same pattern shows up with the drugs that press fewer buttons. In a 24-hour blood pressure study of tirzepatide, the top number fell by about 7 to 11 points over 36 weeks. The researchers said they could not tell if that came from the drug or from the weight loss.
- In lab studies, GLP-1 drugs may also relax blood vessels and help the kidneys let go of salt. That could add a small extra effect.
So the fair summary is: blood pressure dropped, weight loss likely explains most of it, and there may be a little extra on top.
How it compares with semaglutide and tirzepatide
Here is how the blood pressure drop looks next to the two better-known drugs in the same family. Each came from a different trial, with different people and different lengths, so treat this as a rough guide, not a race.
Semaglutide: STEP 1 trial, 68 weeks (placebo -1.1). Tirzepatide: SURMOUNT-1 24-hour monitoring substudy, 36 weeks. Retatrutide: 2025 and 2026 reviews, versus placebo. Lighter shading shows the range. Bars scaled 0 to 12 points.
The takeaway: all three lower blood pressure by a similar handful of points. Retatrutide does not appear to be dramatically different on blood pressure, even though it causes more weight loss. That fits with the idea that most of the drop comes from weight loss, and that the effect levels off.

The heart rate catch
Here is the part many sellers skip. While blood pressure went down, heart rate went up.

The Phase 2 trial described it this way: heart rate rose with dose, peaked around 24 weeks, then came back down. The same thing happens with other drugs in this family.
For most healthy people, a few extra beats a minute is not a big deal. But experts reviewing the Phase 3 results said people who already have a fast or irregular heartbeat need closer watching. That is a real reason trial doctors screen people so carefully.
Can blood pressure go too low?
Yes, and this is the more practical risk for someone who already takes blood pressure pills.
Pills plus weight loss
If pressure falls with weight loss but the pill dose stays the same, the two together can push pressure too low.
Fluid loss
Nausea, vomiting and diarrhea were common in the trials. Losing fluids can make pressure drop, especially when standing up.
Eating much less
Big drops in food and drink intake can leave someone dry without noticing, mostly in hot weather.
Warning signs
Dizziness when standing, feeling faint, blurry vision or a racing heart are reasons to check with a doctor.

What we still do not know
Lower blood pressure numbers are good. But the thing that really matters is fewer heart attacks and strokes. That has not been shown yet.
- No heart outcome results. A large trial called TRIUMPH-Outcomes is testing whether retatrutide lowers heart and kidney events. Results are not out yet.
- Short follow-up. The longest trial results so far run about two years. High blood pressure is a lifelong problem.
- Who was tested. Trial volunteers were screened. People with serious heart rhythm problems or recent heart attacks were mostly left out.
- After stopping. With similar drugs, much of the weight tends to come back after stopping. Blood pressure would be expected to follow.

No drug or research compound replaces the basics. These are the steps doctors reach for first, and they stack on top of each other.
Less salt
Most salt comes from packaged, restaurant and fast food, not the salt shaker.
More plants
Fruit, vegetables, beans and whole grains bring potassium, which helps balance salt.
Move most days
Brisk walking, cycling or swimming lowers pressure within weeks.
Less alcohol
Heavy drinking raises blood pressure. Cutting back brings it down.
Sleep and stress
Treating sleep apnea and getting steady sleep both help.
No smoking
Smoking and vaping nicotine tighten blood vessels and raise heart risk.
Claims to watch for
“Retatrutide causes high blood pressure”
The trials found the opposite. Blood pressure went down on average.
“It replaces your blood pressure pills”
Some trial volunteers cut back, but only under close watch by trial doctors. It is not a swap.
“It protects your heart”
Lower numbers are a good sign, but no trial has shown fewer heart attacks or strokes yet.
“Heart rate goes up forever”
In Phase 2, it peaked around week 24 and then eased off.
When to see a doctor
Blood pressure is a doctor’s job, not a guessing game. Get checked if any of these apply:
- Readings of 140/90 or more at more than one check, or any reading of 180/120 or more.
- You take blood pressure pills and are losing weight fast. Your dose may need changing.
- Dizziness or fainting, especially when you stand up.
- A racing, pounding or skipping heartbeat.
- Chest pain, sudden weakness, trouble speaking or a very bad headache. These need emergency care. Call 911.
Many Canadian pharmacies have free blood pressure machines. A home cuff that is checked against your doctor’s reading is also a good tool.
How to pick a research supplier

Bottom line
Retatrutide did not raise blood pressure in the trials. It lowered it, by a few points on average, and some trial volunteers were able to cut back on a blood pressure pill with their doctor’s help. Most of that likely comes from the large weight loss.
The catch is heart rate, which went up with dose and eased after about 24 weeks. And the biggest question, whether it actually lowers heart attacks and strokes, is still being tested.
Better numbers are a good sign. They are not the same as proof. That is the honest version, and you deserve it before anyone makes big claims about your heart.
