Two different jobs

The easiest way to see the difference is a picture. One works like an eraser on one small spot. The other works on the whole page.

So “which is better” depends on the goal. Someone near their goal weight with one stubborn roll is asking a different question than someone with a lot of weight to lose.

What CoolSculpting is, in plain words

CoolSculpting is a brand name for a method called cryolipolysis. That is a big word for “freezing fat.”

Here is the idea. Fat cells are more sensitive to cold than skin is. A clinic places a cup on your belly, flanks, thighs or chin. The cup sucks the fat in and cools it for about half an hour to an hour. The skin is protected, but some of the fat cells underneath are damaged by the cold.

Over the next few weeks, those damaged fat cells die off. Your body clears them away slowly, like taking out the trash a little at a time.

It was cleared for use in the United States in 2010, and it is now offered at many clinics across Canada.

What the CoolSculpting research shows

A 2015 review in the journal Plastic and Reconstructive Surgery looked at 19 studies of fat freezing. Here is what they found, in plain words.

• Measured with calipers (a tool that pinches the skin fold), the fat layer shrank by an average of 14.7% to 28.5%.
• Measured with ultrasound, it shrank by 10.3% to 25.5%.
• Blood fats and liver tests did not change in any study.
• Side effects were mostly short and mild: redness, swelling, soreness and numbness in the treated spot.

Notice what those numbers are. They measure how much thinner the fat got in the spot that was treated. They are not a drop in body weight. Freezing one belly roll does not make someone lose 20% of their weight.

Many people need more than one session per area to see the change they want.

The rare grow-back risk

There is one rare but serious side effect you should know about. It has a long name: paradoxical adipose hyperplasia, or PAH.

In plain words, it means the treated spot does the opposite of what you paid for. Instead of shrinking, the fat there grows bigger and firmer. It usually shows up 2 to 4 months after treatment. It does not go away on its own, and fixing it often means surgery like liposuction.

The 2025 review also warned that most studies did not follow people long enough to catch PAH, so the real number may be higher. It is still rare. But 1 in 455 is a lot more common than the maker’s early figure of about 1 in 20,000.

What CoolSculpting costs in Canada

CoolSculpting is paid out of pocket. Provincial health plans do not cover it, because it is cosmetic.

Prices vary a lot by city and clinic. Big cities like Toronto and Vancouver tend to sit near the top of the range. Always ask for the total price for the whole plan, not just one cycle.

What retatrutide is, in plain words

Retatrutide is an experimental peptide. A peptide is a short chain of amino acids, the building blocks of protein.

Your gut makes hormones that tell your brain and body when you are full and how to use energy. Think of them as three buttons:

• GLP-1: the “I’m full” button. It also slows how fast the stomach empties.
• GIP: a helper button that works with GLP-1 on appetite and blood sugar.
• Glucagon: the “burn more energy” button. It also pushes the liver to use stored fat.

Drugs like semaglutide press one button. Tirzepatide presses two. Retatrutide is built to press all three. That is why researchers call it a “triple agonist.”

What the retatrutide research shows

Retatrutide has now been tested in two big stages of human trials.

A few more findings worth knowing:

• In the Phase 3 trial, 45% of people in the top dose group lost 30% or more of their body weight.
• In a Phase 2 substudy, liver fat dropped by about 82% in 24 weeks at the higher doses. At the top dose, 86% of people reached normal liver fat.
• These results came from weekly injections given for more than a year and a half, inside a supervised trial.

Retatrutide side effects

Retatrutide’s results are big, and so are its side effects. Most are stomach problems, and they were more common at higher doses.

Which fat each one reaches

Not all fat is the same. There are two main kinds, and this is where the two really split apart.

• Pinchable fat sits just under the skin. It is the kind you can grab. It is the only kind CoolSculpting can reach.
• Deep fat sits inside the belly, around the organs and in the liver. Doctors link it more closely to health problems. CoolSculpting cannot touch it. The retatrutide trials measured big drops in liver fat and in body fat overall.

How long each one takes

CoolSculpting is fast and done. The retatrutide trials were long, steady and weekly. In a trial of semaglutide, a similar drug, people regained about two-thirds of the weight they lost within a year of stopping. So the research does not treat it as a one-time fix either.

Whatever the goal, the basics still carry the most weight: eating enough protein, strength training, sleep and steady habits.

Can they be combined?

People ask this a lot. Here is what makes sense on paper.

• Order matters. Spot treatments work best when weight is stable. If someone is still losing a lot of weight, a freezing session done early may be wasted, since the whole body is changing anyway.

• Pinchable fat must be there. The cup needs a roll of fat to grab. Very lean areas cannot be treated.

No study has tested CoolSculpting and retatrutide together. Anyone thinking about a clinic procedure should talk it through with a doctor.
