Ipamorelin is a five-residue peptide of aminoisobutyric acid, histidine, D-2-naphthylalanine, D-phenylalanine and lysine, C-terminally amidated. Three residues are non-proteinogenic or D-configured, making it a designed construct, not a natural fragment. Tesamorelin is the opposite: the full 44-residue human GHRH sequence with a trans-3-hexenoyl group on the N-terminal tyrosine.
Ipamorelin is most often confused with GHRP-2 and GHRP-6, separate sequences with their own CAS numbers. Tesamorelin is confused with sermorelin, GHRH(1-29) amide with no acyl group, and with the CJC-1295 materials.
The components are studied at two receptors. Raun and colleagues characterised ipamorelin at GHS-R1a in rat pituitary cells, rats and swine, reporting in the European Journal of Endocrinology in 1998 growth hormone release without the corticotropin and cortisol changes seen with GHRP-6 and GHRP-2. Tesamorelin is studied at GHRHR on the somatotroph, a class B receptor read out through cyclic AMP, its acyl group characterised against N-terminal degradation.
Those are measurements by others in cells, animal models and registered clinical programmes. Activity at a receptor in a model does not establish an effect in a person, and nothing is claimed for the pairing.
